Report following audit of prevention of acute spills on Ocean Vanguard

On 21 June 2013 and 20-22 August 2013 - 22.8.2013 the PSA performed an audit of barriers and prevention of acute spills on the Ocean Vanguard mobile facility.


The audit was conducted by means of meetings and interviews with Diamond Offshore and through verifications within selected areas on the facility.

Background
For a number of years, the external environment and barriers have been high priorities for the PSA. These are areas that the PSA considers it especially important for the industry to work on. They have been chosen based on our knowledge and experience of the industry, and are firmly anchored in the risk scenario of petroleum activities.

The audit was founded on the requirements in section 4, concerning risk reduction, and section 5, concerning barriers, of the Management Regulations.

Objective
The objective of the audit was to verify Diamond Offshore's management system and barriers relating to the prevention of acute spills on Ocean Vanguard. Particular emphasis was laid on the management and follow-up of maintenance, well control and operation of equipment.

Results
The audit identified four non-conformities relating to the environmental management system, routines and system for securing loose equipment, labelling of chemicals, and labelling of pipe systems.
