Analysis and investigation of selected well incidents - 2002 - 2006

In the Petroleum Safety Authority's (PSA's) seminar on well safety on 4 May 2006, Professor Bernt Aadnøy of the University of Stavanger reviewed an analysis of five selected well incidents, all of which resulted in reduced well integrity. In the interests of sharing knowledge and experience, the PSA has decided to publish the analyses of well design in their entirety.

Links to analyses of selected well incidents:

Shell / Draugen A 5 (Conductor weakened by corrosion)

ConocoPhillips / Ekofisk W5 - (Failure of hanger for tubing, casing and installation tools)

BP / Valhall A 08B (Significant mud loss in drained reservoir)

Norsk Hydro / Njord (Leaks in production tubing)

Statoil / Snorre P2 (Collapsed casing due to lack of pressure/temperature control in the annulus)

In addition, an audit has been conducted of the planning of the Ormen Lange wells, which is also enclosed to further this sharing of experience.

Norsk Hydro and Shell / Ormen Lange (Challenges associated with lower completion)

The Petroleum Safety Authority Norway has conducted its own investigations of selected well incidents. These have been included in this overview to supplement the overall picture of the causes of well incidents:
