Audit of lifting equipment and lifting operations on West Venture

During the period 9-12 February 2004, the Petroleum Safety Authority Norway (PSA) conducted an audit operation vis-a-vis Smedvig A/S regarding lifting equipment and lifting operations on the mobile drilling installation West Venture.


Background for the audit

The audit was a verification activity concerning the Acknowledgement of Compliance (AoC) which Smedvig had been granted for the installation West Venture (pictured).

The audit was also a follow-up activity in accordance with the overall project assignment, and in accordance with the Storting (Norwegian Parliament) White Paper No. 7 (2001-2002) on health, safety and the environment in the petroleum activities.

The audit focused on the problem areas that emerged in our report "Causal relations in incidents related to lifting operations".

Purpose of the audit

The purpose of the audit was to verify that safety is ensured through the management of technical and operational aspects in planning and implementing lifting operations.

It was also to verify that the technical condition is safeguarded through control and maintenance of lifting appliances and lifting gear, and to make sure that follow-up of undesirable incidents in lifting operations fulfils the relevant requirements.

The activity was to verify the basis for the awarded AoC within lifting equipment and lifting operations.

Result of the audit

The PSA made observations of a technical, operational as well as an administrative/organizational kind. We mention in particular:

We observed deficient transfer of experience. Several of the issues in this report were also pointed out, partly or in full, in the report following the audit of lifting equipment and lifting operations on West Epsilon. This audit was conducted in October 2002.

The training system leaves room for improvement. It is particularly important to maintain everyone's familiarity with safety-critical procedures.
