Audit of management of loading operations from Petrojarl Varg to tankers

Audit: On 3 November 2009, the Petroleum Safety Authority Norway (PSA) conducted an audit of how Talisman Energy Norge AS (TENAS) manages technical, organisational and administrative aspects of loading operations from Petrojarl Varg to tankers.

Teekay Petrojarl Production AS also took part in the audit as owner and operator of the Petrojarl Varg production facility.

Background for the audit
One of the PSA's main priorities is to prevent harm to the external environment, including undesirable incidents and accidents that can lead to environmentally harmful discharges to sea and emissions to air.

The PSA's sphere of responsibility includes measures aimed at preventing such harm, as well as measures that can stop an accident at the source and reduce its duration. 

The joint investigation by the authorities following the oil spill from Statfjord OLS-A on 12 December 2007 and the PSA's investigation after the incident involving the loading hose on Draugen FLP on 10 January 2008, showed that weaknesses in the loading system can lead to discharges of hydrocarbons to the external environment.

This audit was aimed at how TENAS and Teekay Petrojarl Production AS have identified and implemented necessary measures.

Purpose of the audit
The following areas were emphasised in the audit:

  • Organisation and responsibility
  • Risk assessments and system robustness
  • Safety system
  • Maintenance management
  • Management of change
  • Management of incident follow-up
  • Management of emergency response
  • Transfer of experience

The audit was conducted as a meeting in which the players gave presentations regarding these areas and the PSA asked questions for clarification. Verification of governing documents has also been completed.

Result of the audit
We identified nonconformities related to the lack of comprehensive risk analyses covering both the loading system on board the Petrojarl Varg and tankers.

We have also identified improvement items related to exercising the supervisory duty and inspection of tankers, as well as correct contents and correct information in parts of the governing documents for loading operations.
