Flaws in manual elevators

During investigation of a serious incident on the mobile drilling facility West Epsilon in which a piece of 30" casing fell down onto the drillers cabin, our findings included the fact that the type of manual elevator used in this incident did not meet the regulatory requirements. We have informed the industry of this in identical letters dated 21 December 2007.

The construction of the elevator in question does not meet the fundamental principles for safety integration. There were no safety devices to ensure and verify to the user that the elevator was securely closed and locked prior to lifting.

Comparable equipment with similar flaws provided by other suppliers and manufacturers may be in use on the Norwegian shelf.
