Investigation following incident involving personal injury at Stena Don

The Petroleum Safety Authority (PSA) has initiated an investigation following an incident involving personal injury at Stena Don on 6 June 2009. During a lifting operation, a person was hit in the neck by a riser which had loosened and fallen into the drilling area.

The incident occurred at 0905 hours as the mobile drilling facility was working on the StatoilHydro operated Smørbukk Sør field in the Norwegian Sea. The PSA was notified by StatoilHydro at 1037 hours.

The injured person was evacuated by helicopter to the St Olav Hospital in Trondheim.

The goal of the investigation is to clarify the course of events and to identify triggering and underlying causes. The investigation will end in a report which will be published on our web pages.

The PSA’s investigation will primarily target the shipowner, but how the owner and possibly the manufacturer of the equipment discharged their responsibilities must also be clarified.

In addition to its own investigation, the Kristiansund police has asked the PSA for assistance in their investigation of the incident.
