Investigation of undesirable incident on Songa Dee completed

The Petroleum Safety Authority Norway (PSA) has investigated an undesirable incident involving a short circuit in a distribution cabinet on the Songa Dee drilling facility on 29 September 2006. This was an incident that entailed serious personal injury, as well as the potential for additional personal injuries or death. The PSA takes a serious view of this incident, and our investigation has revealed a number of nonconformities.

The incident took place while the facility, operated by Stena Drilling, was working for Norsk Hydro on the Troll field.

Our conclusion is that there has been deficient project management of the work, and we have also identified deficient routines for planning and executing such work. We have noted six nonconformities in relation to the petroleum regulations.

The investigation report is available in Norwegian only.
