Investigation report following accident on 7 May 2009 on Oseberg B

During the dismantling of a scaffold on Oseberg B on 7 May 2009, a worker was seriously injured after falling 14 meters to the cellar deck. He was evacuated by rescue helicopter to Haukeland hospital in Bergen, and later died at the hospital. The Petroleum Safety Authority Norway (PSA) has investigated the incident.

The direct cause of the accident was that the pole collar* did not handle the load and subsequently tore.

During the investigation, we identified non-conformities in relation to the regulatory requirements regarding risk assessment, insufficient planning, insufficient competence, insufficient clarification of management and roles, insufficient governing documents and insufficient maintenance of the supervisory duty.

Based on our findings in the investigation, we have issued a notification of order to StatoilHydro, Aibel AS and the StS Gruppen AS.

Notification of order to StatoilHydro:

Under the provisions pursuant to the Framework Regulations, Section 58, on individual decisions, cf. the Management Regulations Section 3, on management of health, safety and environment, the Framework Regulations Section 5, second paragraph, on responsibility and Section 14, on the qualification and follow-up of other participants, StatoilHydro is ordered to ensure safe working conditions for scaffolding workers during assembly and/or dismantling of scaffolding at facilities which are operated by StatoilHydro. This includes, but is not limited to:

  • Review of framework conditions
  • Establishing of and compliance with governing documents, including requirements for risk assessment, distribution of work in work teams and interfaces between companies in the contract chain.

The deadline for complying with the order is set at 1 February 2010. We must be notified when the order has been complied with.

Notification of order to Aibel AS:

Under the provisions of the Framework Regulations Section 58, on individual decisions, cf. the Management Regulations Section 3, on management of health, safety and environment, the Framework Regulations Section 5, second paragraph, on responsibility and Section 14, on the qualification and follow-up of other participants, Aibel is ordered to ensure safe working conditions for scaffolding workers during assembly and/or dismantling of scaffolding during assignments in the petroleum industry. This includes, but is not limited to:

  • Review of framework conditions
  • Making sure that there are requirements for risk assessment, distribution of work in work teams and interfaces between companies in the contract chain at the location in question
  • Ensuring compliance with governing documents

The deadline for complying with the order is set at 1 February 2010. We must be notified when the order has been complied with.

We have asked the three companies to submit any comments on our notification of order and investigation report by 15 August 2009.

The investigation report includes identified non-conformities in addition to those which form the basis for the notification of order, see Chapter 5.1.3. We have asked the companies for a an account of how these are to be handled. 

The report also contains an observation of an area with potential for improvement, and we have asked for the companies’ assessment of this area. We have asked for a response by 1 October 2009.
