On 25 March 2005 there was an accident on the drill floor in connection with pulling casing in a well on Stafjord C. The derrickman and two roughnecks were engaged in manual work on the bottom hole string (BHS), when this was accidentally loosened from the elevator collar. The BHS fell towards the drill floor, striking and seriously injuring the derrickman in both feet.
The incident had the potential for the loss of multiple human lives. In addition to the derrickman who was hit, two other roughnecks were on the drill floor during the incident. These two were also in the direction of the BHS' fall, and could have been struck.
On the same day as the incident occurred, the Petroleum Safety Authority Norway (PSA) appointed an investigation group which has both assisted the police in their investigation of the incident, and also performed its own investigation. The investigation identified several nonconformities and on 8 June we issued both Statoil as operator and Smedvig as drilling constructor a notification of order. Statoil was issued an order as notified on 5 July.
In accordance with the notification issued, the following order is issued to Smedvig:
Pursuant to Sections 3 and 9 of the Management Regulations, and Sections 22 and 25 of the Activities Regulations, cf. Section 58 of the Framework Regulations, Smedvig Offshore on SFC is ordered to identify and implement measures aimed at management, planning of the work, compliance with governing documents and implementation of activities in accordance with the risk assessment.
Pursuant to Section 11 of the Management Regulations and Section 21 of the Activities Regulations, cf. Section 58 of the Framework Regulations, Smedvig Offshore on SFC is ordered to evaluate the crew's composition and implement actions to maintain individual competence, group competence and distribution of work in relation to the positions of the drilling crew.
Moreover, based on this review, Smedvig Offshore must implement investigations to clarify whether deficiencies disclosed in the drilling operations on Statfjord C are also manifested on other Statoil installations where the company is involved, and to implement necessary actions to improve these conditions, if applicable.
The deadline for complying with the order is set at 1 October 2005. The PSA is to be informed when the order has been executed.
