Order issued to Weatherford following incident on West Epsilon

On 14 September 2007, a section of 30" casing weighing 8.5 tonnes fell onto the drillers cabin on the mobile facility West Epsilon. Subsequent to our investigation, a notification of order was issued to Weatherford Norge AS (Weatherford). The order was issued on 20 December 2007.


The direct cause of the accident was that the elevator was not properly closed and locked when the casing was lifted. The elevator was rented temporary equipment delivered by Weatherford.

On 20 December 2007, we issued the following order to Weatherford:

Pursuant to Section 9 of the Facilities Regulations relating to design of facilities and Section 18 of the Activities Regulations relating to start-up and operation of facilities, Weatherford Norge AS is ordered

  • to ensure that this type of elevator satisfies regulatory requirements so as to limit the possibility of human error, cf. Chapter 5.1.2 of the report,

  • to ensure that satisfactory user instructions are prepared for these elevators and that the user instructions are available in connection with use, cf. Chapter 5.1.3 of the report.

The order relates to all elevators the company has for rental.

The deadline for presenting a final plan with deadlines for implementing the order is set at 18 February 2008.

We must be notified when the order has been carried out.

We regard the notified order concerning implementation of compensating measures, and Item 2 of the notification, as having been complied with.
