Qualitative study of structural and maritime incidents

The Petroleum Safety Authority Norway (PSA) initiated a qualitative study in 2013 to look more closely at structural and maritime incidents on the Norwegian continental shelf (NCS).

This followed a negative trend for reported structural events on the NCS as well as the serious incidents with the Floatel Superior and Scarabeo 8 mobile units in 2012.

Focused on cases which could lead to major accidents, the study concluded that insufficient attention is being paid to the risk potential of structural and maritime incidents.

  • Too few incidents are investigated, investigations are limited in their coverage of key causal conditions, and the safety-critical nature of these incidents is insufficiently appreciated.
  • Information sharing, both between players and between the different phases in a facility’s lifecycle, also ought to be improved.
  • In addition, the status of the structural disciplines involved has been weakened in recent years.
